If you are completing this form as part of the self-review portion of your review, please complete all Employee designated sections. To assist with discussion points during the review process, you are strongly encouraged to provide comments where prompted.
Once you complete your assigned portion of the review form, your supervisor will review your responses and add their scores and comments. The completed form will be discussed in its entirety between you and your supervisor during your scheduled review time. Your review will be approximately 60 minutes in length, but total time may vary.
